4 - Of Remedies and Poisons: Recreational Use of Antiretroviral Drugs in the Social Imagination of South African Carers

Résumé
Au cours d'une étude ethnographique sur les obstacles au traitement antirétroviral (ARv) et sur l'observance de celui-ci
dans la région de la côte ouest de l'Afrique du Sud, notre équipe est tombée sur le sentiment général parmi les
prestataires de soins de santé qu'il existait un commerce illicite animé de médicaments antirétroviraux. en soi, cela est
considéré comme un obstacle à l'observance pour nombre de leurs patients dont les médicaments sont échangés ou
volés par des trafiquants de drogue. des preuves anecdotiques indépendantes émergent à propos de ce commerce, bien qu'il y ait eu peu de données concrètes confirmant l'existence d'un marché récréatif pour les antirétroviraux. Bien qu'il y
ait des rumeurs selon lesquelles l'éfavirenz (dont certains effets secondaires sont hallucinogènes) est utilisé dans le
fabrication de crystal methamphetamine (localement « tik »), de tels rapports, en eux-mêmes, ne semblent pas
capables d'expliquer l'omniprésence (et la confiance) de la croyance dans ce commerce parmi les prestataires de soins de santé avec lesquels nous avons interagi. Cet article explore les aspects du commerce hors indication des ARV (tel que nous le connaissons) et, tout aussi important, comment la rumeur et la connaissance de ce commerce ont gagné en popularité dans l'imaginaire social des travailleurs de la santé et des services sociaux. Cela, selon nous, pourrait précipiter une véritable crise dans le programme de déploiement public du gouvernement.
